    How can you participate?

    Register at the link below select your appointment time and complete the comprehensive survey sent to your email before your appointment date. Attend a recruitment event at the scheduled location and time. Once you complete the health metrics you will receive up to $100 for your participation.

    Who can participate?

    To be eligible for the study you need to meet the following:

    1. Age: 18 years or older

    2. Must have resided or worked in or near the affected area by the Wildfires in Maui in August 2023

    3. Expected to be in Hawaii for at least five years and willing to participate every year

    What to expect on your appointment date?

    Please go to the check-in desk when you arrive. Our staff will help you check-in. They'll make sure your name and birthday match what you put in the survey and on your ID. Remember to bring your ID, or we won't be able to check you in. After you check in our team will guide you on how to give the following samples:

    • Health Measurements. A trained person will measure and record your height weight and blood pressure. They will also perform spirometer readings to look at lung capacity.

    • Urine. You will be asked to provide a small sample of your urine in a clean container. This will help us check on different health markers and heavy metals you may have been exposed to.

    • Saliva. You will be asked to provide a split in a special tube or container to measure chemical compounds and markers of health including the microbiome (natural bacteria in your body).

    • Blood sample. You will be asked to provide about 3-4 tablespoons of blood from your arm. This will help us learn about your health more comprehensively for instance by looking at substances in your blood that can cause inflammation

    Why Share Your Medical Records? Sharing your medical records allows us to gain a comprehensive understanding of the health impacts of the Maui wildfires. By accessing your medical history we can better identify patterns track changes and understand the health effects of the wildfire exposure.

    • Comprehensive Health Analysis: By combining your medical records with the data collected from our health measurements and surveys we can perform a thorough analysis of your health status. This helps us identify any potential long-term effects of the wildfire exposure and create targeted interventions.

    • Personalized Support: Understanding your medical history allows us to offer you more personalized support and resources. This can include tailored health recommendations connections to appropriate healthcare providers and specific interventions based on your health needs.

    How it Works?

    • Simple Consent Form: Participants will allow us to access their medical records by completing a simple consent form with minimal information such as your insurance provider.

    • Established Relationships: We have established relationships with Kaiser and HMSA insurance companies who will securely send us the medical records exclusively for research purposes.

    • Secure Storage: We will store these data on secure University of Hawaii servers.

    • Exclusive Use for Research: The data will only be used for Maui wildfire research and will never be shared with anyone else. No other researchers companies or agencies will be able to access this information.

    Consent Form for Accessing Medical Records: By signing the consent form you authorize the Maui Registry team to securely access your medical records. This information will be used solely for research purposes and to provide you with the best possible care and resources. Your privacy and confidentiality are our top priorities and we will ensure that all data is handled with the utmost care and security.

    • Confidentiality: Your medical records and personal information will be kept strictly confidential and will only be used for the purposes outlined above.

    • Voluntary Participation: Sharing your medical records is entirely voluntary. You can choose to participate or decline without affecting your involvement in other aspects of the Maui Registry.
